Contraindications

Bipolar disorder (may trigger mania). Scheduled surgery. Photosensitivity.

Interactions

CRITICAL INTERACTIONS: Reduces effectiveness of birth control pills, HIV medications, organ transplant drugs (cyclosporine), blood thinners, digoxin, chemotherapy, SSRIs (serotonin syndrome risk), and many other medications via CYP3A4/P-glycoprotein induction.

Side Effects

Photosensitivity (especially fair-skinned), GI upset, dizziness, dry mouth, fatigue, restlessness.

Surgery: Discontinue 5 days before surgery (affects anesthesia metabolism).
Liver: Induces liver enzymes (CYP3A4) — this is the mechanism behind most drug interactions.
